Surgically induced right bundle branch block associated with left antrior hemiblock, complete heart block and/or atrioventricular dissociation


Abstract
From June 1978 to May 1981, 44 patients with ventricular septal defect were treated surgically.
Among the patients, complete pre and postoperative electrocardiographic records were obtainable only in thirty two.
Analysis of the EKG¢¥s revealed four cases of surgically induced right bundle branch block with left anterior hemiblock (12.5%), two cases of complete atrioventricular block and one case of long standing atrioventricular dissociation (8.8%). All but one had had Pp/Ps over 0.45 preoperatively.
Of the `four RBBB+LAHB, LAHB disappeared spontaneously in 3 cases within five .months; the remainder was not followed up so long after discharge.
Since in great majority of our patients, the heart block and/or a-v dissociation developed at operating table, we do not have enough cases of delayed block or delayed recovery to sinus rhythm to allow statistical analysis, interestingly, however, one case revealed delayed recovery to sinus rhythm in 5th month- postoperatively.
